NDIS Support Coordination – Supporting you to understand, implement and use your plan

Understanding what you are funded to receive in your plan can be confusing and overwhelming.

SA Disability Care assists you to understand your funded supports and will work with you to develop your own ability to manage your plan and connect with providers.

Our support workers will help you to increase your capacity to:

  • understand the individual supports you are funded to receive
  • manage service providers in the delivery of their services to you
  • maintain choice and control over the services you receive
  • manage your budget

Our experience with CALD communities means we have strong community connections and can assist with connecting you to culturally appropriate service providers.

SA Disability Care understands that being able to coordinate yourself and book appointments can be hard, but we can help to build this skill with the guidance and assistance of our support workers.
