Nursing Support – Support from our trained health care professionals

SA Disability Care provides home nursing support for the aged and those with a disability who are well enough to be discharged from a healthcare facility, but still require specialist care to further manage medical interventions at home.

Services may include:

  • Medication management
  • Diabetes Management
  • Pressure wound care
  • Wound management

Our nurses work closely with support workers, GPs, specialists, social services, hospitals and other healthcare staff to provide a service tailored to meet your needs.

Nursing Supports Under Home Care Packages

Our Registered Nurses and support workers will work with you to develop a tailored Care Plan, including the input of other, relevant medical professionals to assist clients to achieve their treatment or recovery-specific goals based on their diagnosis. These plans can be therapeutic, rehabilitative, or preventive in nature.

The level of your HCP will determine the funding available for various supports. Learn more about Home Care Package Services here

NDIS Funded Nursing Supports

The NDIS funds capacity building nursing supports such as:
  • Training by a nurse to provide you, a carer, or a support worker to manage things like medication prompting and simple wound care.
  • Assessment by a qualified nurse on what services and level support you need (e.g. a continence assessment).
All NDIS funded nursing services must be related to your disability and assist you to improve your long-term wellbeing. They are usually delivered over a short period of time and stop once your capacity to manage your own care has been built or your short-term medical needs have ended.Nursing care may also be provided to participants who have complex individual support needs that are best met by someone who has medical training and experience. You can receive assistance with these activities if you have funding in your NDIS plan under ‘Improved Daily Living’.  Learn more about the NDIS supports available and their price guide here: 2024/25 Support Catalogue.
