Post Hospital Support – Support for the ageing to assist recovery
SA Disability Care is here to assist you when returning home from hospital so that you may make a full recovery. We can assist with those tasks that are a little harder to complete or which may be dangerous for you during this period (e.g. cooking and cleaning). We are here to make this transition an easy, healthy and safe one.
Our services include:
- Transport home from the hospital
- In-home medication supervision
- Domestic tasks and housekeeping
- Record keeping
- Monitoring your healing
- Attending follow up appointments
Our team will work with you to develop a tailored Care Plan that includes the types of tasks you require assistance with and for how long, to aid with your recovery. The level of your HCP will determine the funding available for these supports. Learn more about Home Care Package Services here.
