Our company is passionate about and committed to delivering effective and high-quality services to meet the needs of our clients. We provide HCP services funded by the Department of Aged Care and through client fees.
Personal Care
Our personal care services are designed for individuals with a Home Care Package. We offer comprehensive and personal home care solutions for you or a loved one to maintain your overall health at home.
Our personal care services cover a variety of activities, ranging from general assistance to more complex needs. These include but are not limited to, Grooming, Showering, Toileting, General Mobility, and Continence Assistance.
Domestic Assistance
Our domestic assistance services are designed to cover everything you need to keep our clients living safely and happily in their own homes. To live independently, anything around the house, from household cleaning to other services such as meal preparation, laundry, and more.
Social Support
We help you stay connected and supported by your local community. As we grow older, we are often more prone to isolation. Therefore, social support keeps you connected to reduce your loneliness. Our support workers can help you attend events and gatherings that will increase community participation and help you meet like-minded individuals with whom to form connections.
Allied health and therapy
We encourage you to access allied health services that suit your care needs and enhance your overall well-being. These supports are often recommended through doctor referrals. They must also be listed on your Care Plan and linked to an assessed care need.
Nursing and Clinical
Our nurses work closely with you, support workers, GPs, specialists, social workers, hospitals, and other healthcare staff to provide a service tailored to meet your needs.
We connect you with skilled healthcare professionals who understand your needs. Our services are flexible, so we can help you maintain your health and stay independent at home.
Respite Care
We’ll support your caregivers so they can take a break from their caring responsibilities. In-home respite care provides a temporary and much-needed break for primary caregivers or family members of Home Care Package recipients. Professional caregivers ensure continuous care and support, allowing caregivers to recharge and tend to their needs.
Transportation
Assistance to travel wherever you need to go, whether it’s a social event or medical appointment that you are attending.
How to Access Aged Care Services
Step 1: Assessment
Start by registering with My Aged Care, the Australian Government’s aged care portal. An assessor will visit you at home to determine your needs and eligibility for services like CHSP or HCP.
Step 2: Choose a Provider
Once approved, you can choose SA Disability Care as your trusted provider. We’ll create a personalised care plan tailored to your goals and preferences.
Step 3: Begin Your Care
